Our Receptionists have been trained as Patient Navigators.


 


The members of our Reception Team have received training to enhance their ability to connect patients directly with the most appropriate source of help, and are now known as Patient Navigators. When patients contact the practice, the patient navigator identifies what their need is, and who will be the most appropriate person to meet their need, whether this be a GP or other option.


 


Services to which patients may be signposted include:


  



  • Minor illness and Practice Nurse Appointments in the practice

  • Pharmacy First 

  • Children’s Primary Care Team 

  • MK health extra appointments at other surgeries 

  • Connect Physiotherapy Service Self-Referral 

  • 111 in order to access an emergency dentist

  • District Nursing and Support Services for carers and older people

If you move to an address inside our practice catchment area please ensure you notify us of your new contact details.

If you move to an address outside our practice catchment area we will ask you to re-register with a practice nearer to your new home.  We have to take this step, as your GP is responsible for your care 24 hours a day.  He or she may be required to carry out an urgent home visit at any time and the further the GP has to travel, the more disruptive this is to the service we provide in the Medical Centre.

It is best to register immediately rather than wait until you are ill.  Thames Valley Primary Care Agency at Reading will send your records to the new practice as soon as you register.
