If you move to an address inside our practice catchment area please ensure you notify us of your new contact details.

If you move to an address outside our practice catchment area we will ask you to re-register with a practice nearer to your new home.  We have to take this step, as your GP is responsible for your care 24 hours a day.  He or she may be required to carry out an urgent home visit at any time and the further the GP has to travel, the more disruptive this is to the service we provide in the Medical Centre.

It is best to register immediately rather than wait until you are ill.  Thames Valley Primary Care Agency at Reading will send your records to the new practice as soon as you register.

You can find the address of other doctors in Yellow Pages.  If you are having difficult finding a suitable surgery, the Thames Valley Primary Care Agency (Tel. 0118 918 3333) should be able to help you.
