Pre-registration with the practice

PreRegistrationWhen you visit the surgery for the first time you will be asked to complete and sign the pre-registration form.  Alternatively you may wish to print off the form, fill it out and bring it with you on your first visit to the practice.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. You may wish to print off the form, fill it out and bring it with you on your first visit to the practice. 

New Patient Questionnaire for Adults

New Patient Questionnaire for those under 14 years of age

From 1 June 2011, when registering as a new patient we will need information to prove your identity and separate information to verify your address.  Please refer to the attached document which lists items that are considered as acceptable identification. 

A birth certificate is also required when registering anyone under the age of 18.

Please note we require the pre-registration form, new patient questionnaire and verification of identity and address to enable us to register you.
