Legs 11

Legs 11 is an evidence-based service providing treatment, health promotion, education and ongoing care for people in all age groups, who are experiencing leg-related problems eg leg ulceration.

Newport Pagnell Legs 11 meets once a week in the Health Education Room.  Attendance is by appointment only.  Transport can be arranged, for a small charge, to and from the Medical Centre and parking is close by.

The Legs 11 staff work in partnership with patients, using best-practice guidelines. They provide a high standard of care in a social, friendly setting, that promotes understanding and peer support. Patients are encouraged to discuss treatment and talk openly about their experience with the nursing team, our valuable volunteers, and other patients who may be experiencing similar conditions. This service is available to registered patients of Newport Pagnell Medical Centre.

Community Nursing

Our Community Matron Team consists of a Matron and an experienced health care assistant.

We look after those who are frail in the community, offering advice and support to the patients and their carers. We support and assess those with memory loss, depression and anxiety, those who are bereaved, those who may find mobility difficult and we try to help our patients access the care that they need. We aim to prevent unnecessary admission to hospital by securing the support in the community. We also work with carers at the practice to advise and support them in the work that they do. Our team supports patients in the long-term so they feel safe and feel comfortable calling should they need help. However this is not an emergency service as there are times when the role is not covered.

Jay McIntosh Community Matron works on a Monday, Tuesday and Friday from 8 am – 4pm

Maggie Adams HCA works on a Monday and Tuesday from 8am – 2pm

District Nurses

The district nursing team are based at Newport Pagnell Medical Centre and also care for Kingfisher Surgery patients and Willen@NPMC.

The team consists of experienced nurses who specialise in providing a wide range of care in the community.  They visit all patients requiring nursing care who are unable to access care at the surgeries.

Their aim is to support patients to enable them to stay in their own homes.  They do this by assessing what care is needed and then co-ordinating and delivering that care.  They also provide a high level of support to the many residential homes and sheltered accommodation in the area.

You can contact the nurses by ringing the medical centre on 01908 611767.

District Nursing Service

  • The District nurses visit patients who are strictly housebound and for medical reasons are unable to attend the Medical Centre for a Practice Nurse appointment.
  • District Nurses are not an emergency service, in case of an emergency please contact your GP or dial 999.
  • All patients referred for continence promotion are entitled to an assessment. Continence is the aim, with continence products used only as a last resort if other appropriate treatment is unsuccessful.
  • Any member of the team may visit patients – this may include staff members that reside locally and bank staff.
  • All packages of care will include health promotion, supporting the service’s overall aim of encouraging greater independence and well being.
